Community Nursing

Community Nursing allows participants to maintain their health and receive hospital level care from the comfort of their homes.

Community nursing supports are available to participants if;

  1. They require these supports due to their disability.
  2. The support is not funded by another health system/service.
  3. The support is value for money.
  4. The need for the support is ongoing.


The NDIS does not cover short term, temporary assistance at home after discharge from hospital or rehabilitation.

Entrust Support Services offers the following nursing services;

  • Medication management
  • Medication administration (administering drugs including sourcing and picking up medication and administering intravenous lines)
  • Assessment of medications
  • Health assessments, advice and education on appropriate safety of equipment for the home
  • Diabetes management
  • Dementia management 
  • Measuring blood pressure, pulse, temperature and blood sugar levels 
  • Training for participants, their families or support workers 
  • Mobility assistance (i.e. transfers from bed to wheelchair)
  • Personal care for people who need hoist assistance (getting dressed, showering, toileting and grooming)
  • Continence assessment and recommendations
  • Wound care

 

These supports are available to participants who have, improved health and wellbeing goals in their NDIS plan and funding under;

Customer Service

We are here to help answer any questions you may have about the NDIS, including eligibility, access, and the support services we offer. Our customer service team is available to assist you Monday to Friday, 8:30am to 4:30pm.

Entrust Support Services has immediate capacity to take behaviour support referrals.
